It is very common for ships to pass between the green marker and the lighthouse as it seems natural if one is not attentive to the direction of the fairway. Additionally, it is easy to conclude here that the fairway leads along the coast and not towards Gothenburg. Last year, I saw several who went on the wrong side and happened to make it, but also groundings.
I also have several examples of sailboats running aground in the area that extends eastward from Smörbådan. It extends further than one might think and there are no markings.
The issue exists in all types of nautical charts.
